I always took GH without insulin.

is it necesarry to add insulin? I heard some people get fat or a small gut sometimes.

So far I see benefits with just GH

Prolonged GH use can reduce insulin sensitivity, especially at higher doses. From people I've worked with, anything over 6iu's or so you'll want to supplement insulin to reduce stress on the pancreas.
