It's always a possibility. Best bet is to focus on the muscle every rep. And don't sacrifice form and body mechanics just to get another rep. When form goes, stop the set. Even when taking a set to failure...go only to positive failure. A partner can give you a nudge for a forced rep or 2. But don't lose form even on those. Be patient. I had only a few muscle tears in 50 years of training...and I trained very heavy...but with control. Momentum is a muscles worse enemy!
