I am 5'8" and was 220lbs fat 38 inch waist. I got down to 174 with keto, cardio, and circuit training. In February I started TRT then began LIFTING. I have also ran a few blasts over the course of the year. I am now sitting at 203 lbs with a 32 inch waist - look totally different.
If you are having blood sugar issues and you are overweight, the answer to all your ills is KETO. You simply have to learn to look at food in a different way.

With the exception of 2 days when I cheated ONCE, I have been in ketosis for a year. My energy levels are better than they ever were. I lift like a mad man. I do my best to eat no more than 30 grams of carbs per day. The first week, when transitioning your metabolism from burning carbs to burning fat, sucks. After that - smooth sailing.

Tren fucked with my blood sugar too. Stop asap. Run trt until you get this figured out. Extended fasting is great for your blood sugar/insulin issues, and your weight loss goals

Basing my opinion off the picture you provided, your body fat is still on the high end of the spectrum. In my experience, it would really be to the benefit of your health to stop running everything except for a TRT dose of Test E or Test Cyp. Once you’ve attained a lower body fat and have your skin surgery, heal up and then reevaluate where you are at and where you want to go.
You could try running a low dose of AI along with a low dose of an estrogen blocker. I am not well versed in dosing with either because I rarely need them. Please start low and gradually increase as needed.
